Halitosis Explained: What Actually Causes Bad Breath

Trisa Dental Solutions graphic explaining halitosis causes, including tongue coating, gum disease, dental decay, dry mouth and non-oral factors

Halitosis is simply the medical word for persistent bad breath. It is extremely common, and it is almost always treatable once the cause is known. The problem is that most people, and many clinics, treat the smell instead of the cause.

For additional information about the causes and management of bad breath, read the NHS guide to bad breath.

If your bad breath keeps returning even after brushing, read our related article: Why Your Bad Breath Comes Back After Brushing.

Oral causes (the majority)

Research consistently finds that most cases of persistent bad breath start inside the mouth.

The main sources are:

  • Tongue coating. Bacteria, dead cells and food debris collect on the back of the tongue. This is the biggest single source in most patients.
  • Gum disease. Bleeding gums, gum pockets and bone loss create low-oxygen spaces that odour-producing bacteria love.
  • Dental decay and failing restorations. Cavities, cracked fillings and loose crowns trap debris.
  • Dentures, bridges and implants that are not cleaned properly.
  • Dry mouth. Saliva washes bacteria away. Less saliva means more smell.

Non-oral causes (the minority)

  • Tonsil stones and chronic tonsil problems
  • Sinus infections and post-nasal drip
  • Some medications that reduce saliva
  • Medical conditions such as uncontrolled diabetes, kidney or liver disease, and rare metabolic disorders
  • Smoking, vaping and certain foods, which are usually temporary

Why the type of smell matters

Sulphur compounds produced in the mouth give a rotten-egg or stale smell. A fruity smell, a fishy or ammonia-like smell, or a musty smell can point to something outside the mouth. We pay attention to this during your assessment.

Imagined bad breath is real too

Some people are convinced they have bad breath when an examination finds none. This is called pseudo-halitosis, and when the worry becomes severe it is called halitophobia. It deserves to be taken seriously, but treating it with lasers would be wrong. Honest diagnosis protects you from unnecessary treatment.

How we approach it at Trisa

We don't start with a product. We start with questions and an examination: how long you've had it, who noticed, what you've already tried, your medical history and medications, and a close look at your tongue, gums and dental work. The treatment plan follows the findings.

For patients whose odour comes from the tongue and gums, the combination of gum cleaning, laser treatment of the tongue surface, and ozone has given us consistent results over many years. For patients whose cause lies elsewhere, we refer.

The key message: halitosis is a symptom, not a diagnosis. Find the cause, and you can usually fix it.
